Before You Report
What to Expect on Your First Day
Prior to reporting, you must complete your new hire forms sent to you through the Entrance on Duty System (EODS). You received several emails containing your User ID, and Password. The forms sent to your EODS account are necessary for us to process your appointment, for you to receive your first paycheck, and for you to finalize legal documents for Federal employment with the National Archives and Records Administration.
Forms to Complete Before You Report and must bring with you on your first day:
Employment Eligibility Verification (I-9)
Who Submits: All new employees.
Purpose: To verify your citizenship and eligibility to work in the United States. Complete Section 1. Print one copy and sign and date the form.
Instructions: On your first day of work, you will be required to provide sufficient proof of citizenship. Refer to the back of this form for a list of acceptable documentation
NOTE: As of July 1, 2010, certified Puerto Rico birth certificates issued prior to July 1, 2010 will be invalid and unacceptable for Form I-9 purposes.
Some of the New Hire forms sent to your EODS account that should be completed before you report are:
Employee's Withholding Allowance Certificate (W-4 Form)
Who Submits: All new employees. If you are a resident of Colorado, Delaware, Idaho, Kansas, Minnesota, Montana, Nebraska, New Mexico, North Dakota, Oklahoma, Oregon, Pennsylvania, Rhode Island, South Carolina, or Utah, we will use the deductions you declare on your W-4 for both your Federal and State taxes. Therefore, if you would like for your state tax withholding allowances to be different from your federal withholding allowances, it will be necessary for you to complete two W-4 forms.
Purpose: Determine the correct amount of Federal income tax to be withheld from your biweekly earnings based on the number of exemptions you claim. Required by the Internal Revenue Service (IRS).
Instructions: Step 1: Use the Personal Allowances Worksheet to help you determine the number of withholding allowances that you are entitled to claim. Step 2: Fill out the W4 Form, and submit through EODS.
Self-Identification of Disability (SF-256)
Who Submits: All new employees.
Purpose: This form is used to collect statistical information for agency reports on hiring, placement, and advancement of disabilities.
Instructions: Complete the requested personal information at the top of the form and indicate the appropriate code and submit through EODS.
Ethnicity and Race Identification (SF-181)
Who Submits: All new employees.
Purpose: This form is used to collect statistical information on the composition of the National Archives and Records Administration workforce.
Instructions: Read and complete the form and submit through EODS.
Statement of Federal Service (SF-144)
Who Submits: All employees with prior Federal service.
Purpose: To help determine and maintain the length and type of previous Federal service.
Instructions: Read and complete the form and submit through EODS.
Direct Deposit Sign-Up Form (SF-1199A)
Who Submits: All new employees.
Purpose: This form is used to identify the financial institution where your biweekly earnings will be forwarded through electronic funds transfer.
Instructions: Read and complete the form and submit through EODS.
Verification of a Military Retiree's Service in Non-War Time Campaigns or Expeditions (SF-813)
Who Submits: All military retirees.
Purpose: This form is used to request verification of a retiree's military service performed in a non-war time campaign or expedition for which badge/medal was authorized, in order to credit such service for leave accrual rate and reduction-in-force purposes.
Instructions: Complete address block and items 1 through 9. Print one copy. Bring this completed form with you on your first day.
