Before You Report

What to Expect on Your First Day

On your first day working at the Treasury Departmental Offices (DO), you will take the Oath of Office and Appointment; you will be required to sign an affidavit affirming this act. You will also be required to submit a variety of documentation. Use the list below or download the First Day Checklist to ensure you have all the necessary items.

Bring These Forms of ID With You

To process your identification verification, please bring all 3 of these original documents:

  • Your Social Security Card
  • Your Birth Certificate
  • Your State-Issued Driver's License, Identification Card, or Passport

Forms to Fill Out Ahead of Time

These forms must be completed ahead of time. Fill them out and bring the completed forms with you on your first day.

  1. Employment Eligibility Verification (I-9)
  2. Declaration for Federal Employment (OF-306)
  3. Employee Address (AD-349)
  4. Employee's Withholding Allowance Certificate (W-4)
  5. State Tax Withholding Form
  6. Self-Identification of Disability (SF-256)
  7. Ethnicity and Race Identification (SF-181)
  8. Direct Deposit Sign-Up Form (SF-1199A)
  9. Uniform Service Status Code
  10. Post-Employment Restrictions
  11. Certificate of Nonresidence in the District of Columbia (D-4A)
  12. Verification of a Military Retiree's Service in Non-War Time Campaigns or Expeditions (SF-813)
